At Hospice & Community Care, we offer more than just medical care—we provide emotional and spiritual support for individuals and families facing the challenges of a life-limiting illness or loss. Our mission is to provide personalized, compassionate care and support for patients and families coping with serious illness through the end of life.

Hospice & Community Care employs a team-oriented approach that combines expert medical treatment with pain management and tailored emotional and spiritual support. We not only focus on the needs and wishes of our patients but also extend our support to their loved ones during these trying times.

Founded in 1980, we are Pennsylvania’s largest nonprofit provider of hospice and palliative care, serving patients wherever they call home. Our mission is to meet the needs of the entire community, offering care 24/7 to anyone in need—regardless of insurance coverage or financial resources. Each day, we care for more than 1,100 hospice and palliative patients across south central Pennsylvania, and support hundreds of grieving families through our Pathways Center for Grief & Loss. Lancaster County represents our largest service area with 60% of our patients and caregivers living here. As a nonprofit, we are often called upon to serve the most complex cases because we will not turn anyone away. Our expertise, specialized resources and community support thanks to you make this level of care possible.

Our core programs include:

  • Hospice Care wherever the patient calls home and at our Bob Fryer & Family Inpatient Center, with 24 private rooms, providing medical, emotional and spiritual support for individuals and families who are coping with a life-limiting illness. Right now, we are caring for 705 hospice patients.
  • Choices Palliative Care provides specialized home-based medical care for people living with a complex serious illness including dementia. Right now, we are caring for 421 palliative care patients.
  • Sunflower Pediatric Palliative and Hospice Care offers specialized care and comfort designed around the unique needs of children with complex serious illness and their families. This includes perinatal specialized care for parents anticipating the birth of a baby with a life-threatening condition. Right now, we are caring for 20 pediatric patients.
  • Veterans Honoring Veterans program recognizes the unique end-of-life needs of veterans in our community. Volunteer veterans visit patients and in a special ceremony provide them with a pin to thank them for their service. Veterans comprise 20% of our patients.
  • Pathways Center for Grief & Loss offers a lifeline for those navigating the pain of losing a loved one—free of charge and open to anyone in the community, whether or not their loved one received hospice care. Services include loss-specific support groups, children and teen programs, and an overnight grief support camp for children. In our current monthly meeting for those under 50 who lost a spouse, participants are grateful to connect with others in their age range and we often hear: “I look forward to coming to this group where I don’t have to explain or justify why I feel as I do.” Last year the Pathways Center served 13,000 people. These services are funded entirely through the generous support of our community as they are not reimbursable through Medicare or insurance, yet they are vital for mental health.
